CAQH login
I'm getting a "something went wrong" error when linking my CAQH account.
- Your CAQH username is not your email address; it's a separate username you created when setting up your account.
- Go to proview.caqh.org and confirm your username (you can reset it if needed).
- Once confirmed, re-enter your credentials in the intake form.
I'm seeing a "re-attestation needed" message.
- Log in to your CAQH ProView account.
- Click the Review & Attest button at the top right of your profile.
- Once re-attested, return to the intake form and click Recheck CAQH info.
If you recently completed your CAQH profile for the first time, allow up to 48 hours before continuing.
My CAQH profile says "profile data submitted" but it's still not working.
- CAQH typically takes 24-48 hours to process a newly completed profile.
- If it's been over 48 hours, log in to CAQH and check for any errors or missing documents on your profile.
- Once all issues are resolved and your profile is approved, return to the intake form and refresh.
License
The intake form says it couldn't find my license.
- Double-check that your license type, number, and state are entered correctly in your CAQH profile.
- Make sure the license number matches exactly (including any leading zeros or hyphens) as shown on your actual license.
- Update your CAQH profile if needed, re-attest, then click Recheck CAQH info in the intake form.
The intake form says the name on my license doesn't match.
- The name in your CAQH profile must match the name on your license exactly (e.g., maiden name vs. current name).
- Update the name in your CAQH profile to match, then re-attest.
- Click Recheck CAQH info in the intake form.
My license has been stuck in "verifying" for a while.
- Confirm your license type, number, and state are entered exactly as shown on your license.
- If everything looks correct, continue filling out the rest of the intake form.
- Contact your Headway Practice Consultant directly; they'll follow up to help resolve the verification issue.
Malpractice / insurance
My malpractice coverage doesn't meet the required limits.
- Check your Certificate of Insurance (COI) to confirm your per-occurrence and aggregate coverage amounts.
- If your coverage is below the required minimums, contact your insurance provider to increase it.
- Once updated, add the new policy to your CAQH profile and re-attest, then refresh the intake form.
My policy isn't active yet or has already expired.
- Your policy must be active today to proceed.
- If it starts in the future, contact your insurer to adjust the start date.
- If it's expired, obtain a renewed policy, upload it to CAQH, re-attest, and refresh the intake form.
My policy is expiring soon.
- You'll need to provide a renewal policy that covers the period after your current one expires.
- If you don't have a renewal yet, contact your insurer to purchase one.
- Upload the renewal to CAQH, re-attest, and add it to the intake form.
The insurance doesn't list me as individually insured.
- Your COI must list your name under Owner or Name Insured.
- If only your LLC or practice name is listed, call your insurer and request an addendum that includes your name.
- Upload the updated document or addendum to the intake form.
I uploaded my insurance document but it's being rejected.
- Make sure you're uploading a Certificate of Insurance (COI) – not a declarations page.
- The COI should say "Certificate of Insurance" at the top of the page.
- Resubmit the correct document in the intake form.
Education
My education credentials aren't being accepted.
- Log in to CAQH and confirm your degree was added under Professional School, not undergraduate.
- Check that your start and graduation dates are correct — the graduation date cannot be before the start date.
- Make sure you selected Yes for "Did you complete your professional education at this school?"
- Save your changes, re-attest, then click Recheck CAQH info in the intake form.
Employment
I'm being asked to update my employment information in CAQH.
- Log in to CAQH and add employment history for any gaps in the requested time period.
- If you're currently employed with no end date, mark that position as current.
- If you had a period of unemployment, add an explanation for unemployment in CAQH.
- Save your changes, re-attest, then click Recheck CAQH info in the intake form.
Practice location
I'm getting an error about a missing practice state.
- Log in to CAQH and add the missing state(s) under your practice locations.
- Re-attest your profile.
- Return to the intake form and click Recheck CAQH info.
Certifications
I'm a nurse practitioner and I'm being asked to add board certification.
- Log in to CAQH and go to Profile Data > Personal Information.
- Set your NUCC Grouping to "Physician Assistants & Advanced Practice Nursing Providers." Save and continue.
- Go to Profile Data > Specialties and select the appropriate psychiatric/mental health specialty.
- Select Yes under "Board Certified?" and choose American Nurses Credentialing Center (ANCC) from the certifying board dropdown.
- Complete the remaining fields, save, and re-attest your profile.
This requirement applies to all nurse practitioner license types.
