Self-funded plans covered under our provider agreement with Quest Behavioral Health

Below is the list of self-funded plans covered under our provider agreement with Quest Behavioral Health. This may expand in the future. 

  • ASO Employer Groups
    • University of Pennsylvania (PennCare PPO plan only)
    • University of Pennsylvania Health System (PennCare PPO and CDHP plans only)
    • Tower Health
    • WellSpan Health
    • Penn Medicine Lancaster General Health

* Quest Health does not issue cards for the behavioral health carve-out for these plans, however, Quest should be noted on the back of a member card with their telephone number.
